Congratulations on being selected in the Sydney East U/10 Narellan Carnival teams in 2026. Please confirm your place in the team by completing this form. In doing this you are required to:

  1. review and acknowledge the SEHA Junior Player and Parent Code of Conduct and Medical Waiver
  2. Complete the Medical Questionnaire for the competing player
  3. Make payment of the Rep Fee of $30 (including transaction fees of $2.20) in accordance with the payment instructions.  

This payment covers the costs SEHA incurs to enter the team into the MSW Narellan Carnival, as well as providing training venue hire. If you are not able to make payment for any reason please contact repconvenor@seha.com.au to discuss special consideration. Any discussion will be treated with the utmost confidentiality.
